Check the person and the pharmacy

A neat website can tell you very little about who is doing the work. Ask for names you can match to public records.

In Surprise, start with Arizona’s boards

Get the clinician’s complete name and ask for the number and type of their licence. The letters after a name help you choose the right board. Search the person, rather than just the business name.

MD and DO

MD means medical doctor. Arizona’s Medical Board lets you search an MD’s licence and read the board’s public decisions about that doctor.

Doctors whose names end in DO have a different Arizona licensing board from doctors whose names end in MD.

Use that separate board’s search for a DO.

Read any decision you find. Ask the board to explain a restriction you do not understand.

Nurse practitioner and physician assistant

Arizona’s Nursing Board has online checks for a nurse’s licence and a separate certificate for work as a nurse practitioner.

For a nurse practitioner, check both the nursing licence and a certificate naming that role.

A nurse practitioner must have permission from the Nursing Board to prescribe, and prescribing must stay within their training.

PA means physician assistant; Arizona’s PA board offers its own licence search.

Ask the Nursing Board to confirm whether that person has permission to prescribe the medicine you are considering.

An out-of-state clinician needs a separate check

Arizona telehealth registration

Some out-of-state clinicians can treat a person in Arizona through registration with the matching Arizona board.

This route requires a qualifying licence in another state with no practice restrictions. Arizona also sets rules for past discipline, insurance for claims about their care and reports to the board. Past discipline means action a licensing board took over the clinician’s work. Registration allows care under these remote-care rules; it is not a full Arizona licence.

Ask the matching Arizona board to confirm that the registration allows this person to treat you here.

Check the pharmacy too

Permit

Permit is Arizona’s name for a pharmacy’s state permission.

State rules say a pharmacy shipping into Arizona from elsewhere must hold permission from its home state as well as an Arizona permit.

A pharmacy permit alone does not show that it may legally make the specific medicine being offered.

Get the pharmacy’s actual name. If the online permit record is unclear, ask Arizona’s Pharmacy Board to verify it.

Limits of an online record

Only some board records appear on Arizona board websites. Further public records can be requested from the board.

An empty list of board decisions does not prove there was never a complaint or action.
